MDS Assessment Coordinator (RN) – Full-Time | $45/hr | 8 AM – 4 PM (this position is NOT REMOTE)

**Must have Minimum of one year of experience as an MDS Assessment Coordinator in a long-term care setting.

Hamilton Nursing and Rehab is seeking a dedicated MDS Assessment Coordinator (Registered Nurse - RN) to join our team at our 64-bed skilled nursing facility in Detroit, MI. This role is essential in ensuring accurate and timely resident assessments in compliance with state regulations and facility policies.

Position Details:
Schedule: Full-Time, 8:00 AM – 4:00 PM
Compensation: Starting at $45/hour
Key Responsibilities:
Conduct routine resident assessments per pre-determined schedules and as needed for condition changes, hospital returns, etc.
Accurately observe, assess, and communicate any changes in residents’ conditions to the appropriate personnel.
Ensure timely completion of assessments and Care Area Assessments (CAAs) by Social Services, Activities, Dietary, and other departments, obtaining required signatures.
Complete MDS, quarterly assessments, CAAs, and care plans as needed.
Develop and update individualized care plans based on residents' needs.
Coordinate and distribute the MDS assessment schedule to the interdisciplinary team.
Transmit MDS assessments to the state as required and maintain accurate documentation, including transmittal logs.
Qualifications:
Active Registered Nurse (RN) license in the state of Michigan.
Minimum of one year of experience as an MDS Assessment Coordinator in a long-term care setting.
